Information about registration
Our practice is currently open for new patients. A few conditions apply to this process. Firstly, in the event of an emergency, the GP must be able to arrive at your home address within 10 minutes. For this reason we can only register patients who live within our postal code area:
– Postal codes 5614, 5643, 5644, 5645, 5646 in full;
– Postal codes 5611, 5613, 5615 up to the Dommel and the Canal.
Secondly, your current GP must be located outside the Stratum district.
Deregister with your previous GP
Let your previous GP know that you are transferring to our practice. For example, send a copy of your registration form and agree (in writing) to sending over your medical file.
WID registration
We are obligated by law to do a so-called ‘WID’ registration check for every newly registered patient. WID stands for ‘Wettelijke Identificatie’ (Legal Identification). We therefore ask you to come by the practice with valid identification. Please bring a copy of the ID of fellow family members with you.
For children who do not have an ID card yet, please bring confirmation of registration in the municipal personal administration (GBA) with you. If you have Dutch citizenship and your child was born in the Netherlands you will have received this after the birth declaration. It contains the social security number (BSN number).
If you are unable to come to the practice for the WID registration, it is also possible to send a copy of your identification by email.

Landelijk schakelpunt (LSP)
In case you need medical attention in the evenings, nights or weekends you can call the central GP post. General practitioners can share important data about your health with the central post through the national exchange service called ‘Landelijk Schakelpunt’. Exchange is only allowed if it is necessary for your treatment and only if you have given permission for this.
You can give consent for the exchange of your medical data at www.volgjezorg.nl. On our registration form you can also indicate whether you do or do not give permission for sharing your medical data with the central GP post.
MijnGezondheid.net (MGN)
MijnGezondheid.net is an online health service connected to your doctor and pharmacy. With this service you can schedule appointments, order medicines or do online consultations. If you want to use mijngezondheid.net, you can indicate this on the registration form.
Privacy and personal data
We process your data in accordance with our privacy and data security policy. You automatically agree to this when you sign up as a patient with us. By signing the registration form you indicate that you have taken note of your rights and that you agree to our processing.
This is how we handle your personal data:
– We store and use your personal data so that we can provide you with good health care.
– We only share your data with third parties if this is necessary to provide good care. If specific permission is required by law, we will ask you in advance.
– We process your data based on the treatment agreement as described in the Medical Treatment Agreement Act (WGBO).
– You have the right to inspect your medical file. Submit a written request to the practice for this.
– If you believe that the data we have stored about you is incorrect, you may put in a request to change your data. Submit a written request to the practice for this.
– You can ask us to delete your personal data. You can also withdraw your consent to the processing of your personal data. If you choose this, we may no longer be able to provide you with responsible care. The WGBO requires a retention period of 15 years. Therefore, in this case we will not delete your data, but label it as ‘inactive’.
– You can object to us if you do not agree with the way we process your personal data.
– You will receive a message from us if something has gone wrong with your personal data.
– We do not keep your data for longer than necessary to provide good care. We adhere to the WGBO-indicated time period.
– All of the above also applies to personal data that we receive through third parties.
